TERMS
1. For each analysis sample, the Analysis Request Form for the relevant device must be filled out completely. After the Analysis Request Form is completed and signed on the computer, it must be sent to GUBITAM.
2. The customer is responsible for taking samples in accordance with the analysis conditions, transportation, packaging and storage until the sample is accepted to the laboratory. In addition, all postal/cargo costs are the responsibility of the customer.
3. By filling out and signing the Analysis Request Form, the customer declares the harmful effects of the sample sent for analysis (if any) on human and environmental health, and accepts that he/she will be responsible for any nonconformities that may occur if not. If an inappropriate sample is sent, GUBITAM has the right not to accept the sample.
4. Samples must be brought in a package that will not spill or leak and will not be affected by the container in which it is placed, and there must be a legible name or code number on the package that identifies the sample.
5. If a sample requires preprocessing before analysis, it should be discussed with the relevant analysis supervisor in advance.
6. Any special conditions related to sample and analysis requirements or methods should be clearly stated in the relevant application form.
7. If analysis samples are requested back, storage conditions must be specified. Otherwise, no special precautions will be taken for storing the samples. Samples and analysis results are stored for 15 (fifteen) days from the report date. Unclaimed samples will be disposed of as waste.
8. The list of tests, their fees, sample acceptance conditions and payment terms are published in detail on the website (https://gubitam.gop.edu.tr).
9. The estimated analysis time is 15 working days after sample is receipt. Declared analysis times are estimates and GUBITAM cannot be held responsible for any delays that may occur due to unforeseen reasons. In case of deviation from the promised conditions, the customer is informed in writing or verbally.
10. Analysis results are sent digitally to the customer e-mail address.
11. If the customer disputes the results of the test, and the same results are found upon repeating the test, the full service fee will be charged to the customer.
12. In cases of dispute, Tokat Courts have jurisdiction.
